Please note that we cannot accept requests on paper for repeat medication due to risk of cross infection. This is only whilst we are going though the Covid-19 pandemic.
**This includes any pharmacist requesting on your behalf** .
It my be useful, to help your pharmacist, to register for online access to order your own repeat prescriptions and then your prescription will go electronically to your nominated pharmacy.
Please contact us for a linkage key to get access online.
NB: it is your responsibility to check with your pharmacy that your prescription has been ordered.

We have made a slight change to our prescription form. The form is very similar to the existing form and self explanatory but you will need to delete any bookmarks you have to the existing form as it will no longer be available. Any details you have saved will no longer available
You may order a repeat prescription by
- Handing in your request at the surgery
- Posting your request with a SAE
- Online via the link at the top of this page
- Ask a local pharmacy to request on your behalf
Please note we require 48 hours' notice for repeat prescriptions.
If your chemist delivers your prescription to your home please arrange with your chemist to pick up the prescription from the surgery.
The information at the bottom of your prescription is important; please check if the message applies to you.
We prefer to review long-term medication after a few months depending on your health problems and therefore request that you attend the doctor in person from time to time.
